The plan includes:
- Annual physical
- 2 sick visits per year
- Additional sick visits for $55 each
- Quarterly visits for chronic disease management, including diabetes and hypertension
- One additional X-ray, such as an abdominal or joint X-ray
- Guaranteed same-day sick visits
